The Department of Anatomy and Cell Biology is taking steps to assist student聽who need to request to defer a mid-term exam or other in course assessments, when no other supporting documentation is available.

Following the Quebec government announcement聽to reduce the strain on an already taxed medical system,聽students are no longer required to provide a doctor鈥檚 note for absences of fewer than five days for聽incidental illness.聽

Please note supporting documents are required for absences as a result of illness where the absence is for 5 or more day and聽is still required for non-medical requests. Valid documentation is to be provided to the聽Course Coordinator within FIVE working days of the original exam date.聽

The decision to approve or deny a request rests with the Course Coordinator. 2) Making a false statement on this form is considered a violation of the Code of Student Conduct and Disciplinary Procedures, Section 19, Misrepresentation of Facts (a) 鈥淚t shall be an offence to misrepresent material facts to another for the purpose of gaining admission to the University or gaining academic advantage or credit鈥� and will be reported as an academic offence to a Disciplinary Officer. 3) All submitted forms will also be kept on file at the Anatomy & Cell Biology department office for future verification.
