Chronic disease thro ughout the life course has a\nsignificant impact on the aging process and is most prevalent in\nlater years. The cumulative effect of chronic diseas e throughout\nthe life course and the age-related decline in physiological \nreserves contribute to the onset of frailty\, disability and\ndependency in the aging population\, and it is widely recognized\nthat chronic disea se is the most important cause of illness and\nmortality burden. In fact\, chronic disease has become the leading\ndriver of healthcare resource uti lization and constitutes an\nimportant burden for our society.\n \nAging a nd chronic\ndisease are therefore two key issues in the challenge to impro ve\nthe health of the Quebec population\, delay the onset of frailty and\n dependency\, and promote independent and healthy aging.
